Referrals

At ALD Support we welcome referrals from health and social care professionals.

If you yourself, or you are a parent/carer of someone who requires support, please contact us for more information.

Who we support

We support:

  • People with a primary diagnosis of Learning Disability and/or Autism Spectrum Disorder.
  • Aged 16+.
  • Support in the following areas – Surrey, London, Dartford and Hampshire.

We provide outreach support and supported living. You can find more information about this support here.

What happens when I refer someone for support?

When you make a referral to ALD Support we will process the referral and identify if we have capacity to support. If we can, we will then proceed to booking an initial assessment.

At the initial assessment we will arrange to meet with you and who is important to you to discuss your needs and what support you are looking for. This usually takes between 1-2 hours.

We will ask questions around topics such as your diagnosis, medication and what support you need. It is also an opportunity for you to ask us questions as well. After the assessment we then decide if we can support you and talk to your social worker and/or case worker.
